＊倫理審査が必要な研究であるという誤解を避けるため、タイトルを「AIを使用した実践報告のお知らせ」に改めました。

【実施概要】

当院では、2025年9月2日より患者さんの安全を最優先に、AI（人工知能）を補助的に活用しながら、重篤疾患の見逃しを防ぐための「レッドフラグ一次スクリーニング」を実施しています。
AIは説明文の作成や記録の標準化を支援するために用いられ、最終判断と説明責任はすべて担当鍼灸師にあります。
AIの利用を望まれない場合は、オプトアウト（不参加）が可能です。→９月末をもって一度締めきりました。ご協力ありがとうございました。

【実施ルール】

対象：新規の「腰痛／頭痛／しびれ／めまい」で受診された方（複数該当可）
タイミング：受付〜問診直後、施術前の約5分で実施
内容：ガイドラインに基づくチェック項目で、緊急受診が必要なサイン（レッドフラグ）を確認

【結果説明】

該当あり → その日のうちに整形外科・脳神経内科・救急への受診を推奨（鍼灸施術は原則いったん保留）。必要に応じて紹介状を作成します。
該当なし → 施術可。ただし症状が悪化した場合は再評価します。

【記録の透明性】

チェックに用いたAIプロンプトと出力の「実行時刻」をメモし、当日中にPDF保存します（カルテの補助記録）。
記録は個人情報を保護した形で安全に保管します。
このスクリーニングは診断そのものではなく、鍼灸を安全に行うための一次チェックです。必要に応じて早期に専門医へつなぐことも目的としています。

【オプトアウト方式について】

当院では、スクリーニングの運用実態を明確にし、安全性をさらに高めるため、診療実践の記録と品質改善の取り組みを行っています。

・記録・保存するのは個人を特定できない集計情報のみです。
・AIの使用と記録は、透明性と再現性を確保するために行います（プロンプト・出力のPDF保存など）。
・この取り組みへの参加は任意であり、オプトアウト（不参加）をいつでも選べます。

オプトアウト方法：受付または担当鍼灸師に「AI利用を希望しません」とお伝えください。
メールやお電話での申し出も可能です。
オプトアウトしても施術の内容や質に不利益が生じることは一切ありません。

【プライバシーとデータの取り扱い】

記録は匿名化し、院内で安全に保管します。
今後、必要に応じて活動内容を報告・共有する場合も、個人が特定される情報は一切含めません。

【よくある質問（FAQ）】

Q. AIは患者情報を勝手に使いますか？
A. いいえ。匿名化・限定利用であり、最終判断は人間（担当鍼灸師）が行います。

Q. AIを使ってほしくないのですが？
A. オプトアウトできます。受付・担当鍼灸師までお知らせください。

Q. なぜ保存（PDF化）するのですか？
A. 説明責任と再現性（透明性）のためです。いつ・どの説明をもとに判断したかを残し、患者さんの安全に資するために記録します。

To avoid misunderstanding that this activity requires formal research ethics review, the title has been revised to “Notice on AI-Assisted Clinical Practice Report.”

Overview

Since September 2, 2025, our clinic has been implementing a “Red Flag Primary Screening” to prioritize patient safety and prevent missed diagnoses of serious conditions.
AI (artificial intelligence) is used only as a supplementary tool to help standardize documentation and generate explanatory text.
All final clinical decisions and responsibilities remain with the licensed acupuncturist.
Patients who do not wish to participate may opt out at any time.

Implementation Rules

  • Target patients: New visits for low back pain, headache, numbness, or dizziness (multiple conditions may apply)

  • Timing: About 5 minutes during intake and interview, before treatment begins

  • Content: Guideline-based checklist items are used to confirm whether urgent referral (red flags) is necessary

Result Explanation

  • If red flags are present: Immediate referral to orthopedics, neurology, or emergency care is recommended (acupuncture treatment is usually deferred). Referral letters may be provided if needed.

  • If no red flags are present: Treatment may proceed. However, symptoms are re-evaluated if they worsen.

Transparency of Records

  • AI prompts and outputs, along with execution time, are noted and saved as PDF on the same day (as supplementary clinical records).

  • Records are stored securely with patient privacy protected.

  • This screening is not a diagnostic act; it is a safety check to ensure acupuncture is performed appropriately and to enable timely referral to physicians when needed.

Opt-Out Policy

To clarify the actual operation of this screening and further improve safety, our clinic conducts quality improvement activities based on anonymized practice records.

  • Only non-identifiable, aggregated information is recorded and saved.

  • AI use and record-keeping are conducted to ensure transparency and reproducibility (PDF storage of prompts and outputs).

  • Participation is voluntary, and patients may opt out at any time.

How to opt out: Please inform the reception desk or your acupuncturist, saying, “I do not wish AI to be used.”
Requests can also be made by email or phone.
Opting out will not affect the content or quality of care you receive.

Privacy and Data Handling

  • All records are anonymized and securely stored within the clinic.

  • If activities are reported or shared in the future, no personally identifiable information will ever be included.

Frequently Asked Questions (FAQ)

Q. Does AI use my personal information without permission?
A. No. All data are anonymized and used in a limited way. Final decisions are always made by a human acupuncturist.

Q. What if I don’t want AI to be used?
A. You may opt out at any time. Please notify the reception desk or your acupuncturist.

Q. Why are records saved as PDF?
A. For accountability and transparency. Saving records shows when and how decisions were made, helping to ensure patient safety.
